Chronic paronychia, by contrast, endures beyond six weeks and results largely from repeated exposure to irritants and moisture, weakening skin defenses. Although often fungal co-infection is involved, persistent bacterial colonization perpetuates inflammation and discomfort.

Diagnosis primarily depends on clinical evaluation, including inspection of the affected nail and surrounding tissues, along with a detailed history emphasizing potential risk factors such as occupational exposure or personal habits. When pus or drainage is present, culture and sensitivity testing can identify the specific bacterial pathogen to tailor antibiotic therapy. In rare severe or recurrent cases, imaging techniques like ultrasound may be employed to assess deeper tissue involvement or abscesses.

Treatment and Management

Early-stage bacterial nail infections often respond to conservative measures such as warm water soaks several times daily to encourage drainage and reduce inflammation. However, if pus accumulates or symptoms escalate, medical intervention becomes necessary. Antibiotic therapy targeting Staphylococcus aureus and other typical pathogens constitutes the mainstay of treatment. Topical antibiotics may play a supportive role in mild cases.
For abscesses, incision and drainage procedures facilitate symptom relief and expedite recovery. Importantly, chronic or recurrent infections warrant addressing underlying causes, including avoiding irritants, improving hand hygiene, and protecting nails from trauma or moisture.

Preventing bacterial nail infections involves minimizing factors that compromise the skin barrier. This includes avoiding aggressive manicuring, refraining from nail biting, protecting hands from prolonged water exposure, and using protective gloves when handling irritants. Maintaining nail hygiene and promptly treating any minor injuries around the nail can significantly reduce infection risks.
